Advance Medical Directive (AMD)
An Advance Medical Directive (AMD) is a legal document that communicates to your doctors that you do not wish to have extraordinary life-sustaining treatment in the event you become terminally ill and unconscious. Extraordinary life-sustaining treatment refers to measures that will prolong your life but not lead to any meaningful recovery.
Making an AMD is a voluntary decision. It is entirely up to you whether you wish to make one.
The AMD needs to be completed in the presence of two witnesses, one of which must be a doctor. Our doctors will explain the nature and implications of an AMD to you and ensure your understanding before we witness it.
How we can help you make a AMD
You will need to be at least 21 years old, and have mental capacity

Make an appointment with our clinic
Bring your NRIC to our clinic
Once completed, you will have to mail it to the Registrar Of AMD
The Registrar will send you an acknowledgement when the AMD has been registered
